The registered nurse will assess your clinical needs and in collaboration with you and the relevant professional(s) draw up a suitable care plan to meet your care needs and we can help you lead an independent and healthy life.

Community Nursing can be provided in the comfort of your home by our qualified nurses and their services can be accessed on a regular or occasional basis.

Our Innovative Community Participation service is tailored to best suit the participant, and their goals, to allow them to get the most out of the service

Household Tasks

Patience Care Services provide assist with household tasks to allow the participant to become more active to handle their daily tasks on their own or with minimal assistance. We support to enable you to live as independently as possible. and ensure that you live in a safe, healthy environment where you are comfortable and independent at all times.

We work with you to ensure that your home is cared for just the way you like it. Our services are available on a regular or occasional basis, depending on your choice.

Plan Management

Plan Management is a service that Patience Care Services offers to assist you with all the financial tasks associated with your NDIS plan. Our plan management service focuses entirely on strengthening the NDIS participant’s ability to manage the financial management of supports.

We help you to understand your NDIS plan and its budget and assist you with finding the right support and services to help you achieve your goal and manage it. We ensure that you get the most out of your plan without you having to take on the administrative burden.

Our Plan Management includes the following services:

  • Help you to improve financial and plan management skills
  • Help you to Manage monthly budget
  • Making payments to providers
  • Planning monthly statements
  • Claiming for payment from the NDIS portal
  • Your Annual Plan Review

Therapeutic Supports

Our experienced supports team provide specialist therapeutic supports to participants with an approved NDIS plan will support you with information, medication management, motivation, counselling, care coordination, and care escalations. We are able to provide assessment and tailor an approach to treatment that helps individuals achieve their goals.

The following are included in our NDIS Therapeutic Support Program:

Occupational Therapy – will help you to develop your functional skills to improving your day-to-day participation and independence. This will also include assessments for daily activities such as showering and cooking.

Behaviour Support – is designed to help you achieve your goals surrounding your mental health, behaviour and relationships. Assistance is provided so that you, your family and or your carers can implement positive behaviour strategies.

Speech Therapy – Speech pathologists assist you with communication. Speech pathologists treat communication disorders, including difficulties with speaking, listening, understanding language, reading, writing, social skills, stuttering and using voice.

Physiotherapy – Physiotherapy is treatment which can restore, maintain, and make the most of a patient’s mobility, function, and well-being.

Support Coordination

Patience Care Services is a NDIS disability service provider in Melbourne, our services are planned to fit your individual needs and goals and take your circumstances and levels of complexity into consideration. We work with you and your family to build capacity so that we can work together to make you more independent.

Support Connection – This support service builds your ability to connect with informal, community and funded supports – Generally provided by the LACs

Support coordination – This support service includes a support coordinator who will work with you to ensure coordination of various supports to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and participate in your community.

Group/Centre Activities

Patience Care Services has Centre-based Activities and Groups to promote socialisation. These Centre-based Activities are facilitated by support workers and has self-directed skill building activities. our participants the choice of undertaking activities independently or with the assistance of a worker. The range of activities we provide is restricted only by our participant’s imagination.

We support you as you venture out into the community to get involved in local community groups, centers, events and clubs. We want you to feel confident by learning new skills and developing old ones
